Treatment

• Identify, establish, and generate awareness and use of directories of community-based physicians and health facilities to increase knowledge of and access to available community healthcare resources.

• Explore feasibility of establishing a centralized, community-based facility for scheduling appointments.

• Establish on-going relationships with “patient navigator” programs arising from the Patient Navigator, Outreach, and Chronic Disease Prevention Act passed in 2005 to provide better access and higher quality healthcare to low income Americans.
